We will bill your visit directly to your insurance provider if you supply an updated proof of insurance. We are happy to file the claim for you, but the balance is your responsibility if you insurance company or plan does not pay after 30 days. Any amount not covered by your insurance policy is due immediately after you receive your first invoice from us.
We will do our best to provide you with co-insurance and cost information prior to your surgery. You may be asked to pay your co-insurance or deductible amount prior to your surgery. Please let us know if you need to set up payment options.
Medicare patients: As a courtesy we will bill your secondary insurance provider directly. You will be asked to sign a Advanced Beneficiary Notice (ABN) for services prior to any procedure. Which states if Medicare does not pay for your procedure you understand that you are responsible for covering these services.

***As each policy is different it is the patients responsibility to know if they need a referral or prior authorization to be seen in this office. Please check with your carrier if you are unsure.
